  • FILL IN THE FOLLOWING BLANKS:

d. You are the Applicant so fill in your name and address

e. Fill in “GEORGIA” in the Constitution and Legislature areas of the document

f.  Sign your name in “Autograph” and date

  • Scan the signed Notice with a scanner or a phone app. If you do not have access to a scanner, we recommend downloading Adobe Scan on your phone. Once scanned with Adobe Scan, you can share a copy directly from the app.

Mail via US Postal Service the ORIGINAL ALONG WITH THE CERTIFIED MAIL RECEIPT TO: DAWN M JONES of the STATE BAR OF GEORGIA and COPIES to Governor Brian Kemp 206 Washington Street, Suite 203, State Capitol,
Atlanta, GA 30334, Georgia House (ALL)  and Senate (ALL).

  • Email a copy of your scanned notice and certified mail receipts by attaching them to your email and address your email accordingly:
    In the TO field the President of the Bar- Dawn M. Jones djrnlaw@aol.com

    In the BCC field copy and paste the email addresses below.  This will send the Notice as an individual e-mail to each House/Senate Member, AG Carr, Judge Brian Amero, Fulton County attorneys, Fulton County Board of Elections, and the attorneys for the Fulton County Board of Elections.
