Affidavit of Demand to Decertify Election Results based on Maladministration Case Number: PGR-2021.11.3

  1. YOU WILL PRINT OR DOWNLOAD AND PRINT THE DOCUMENT ON 8 ½ X 14 LEGAL PAPER SEVEN (7) COPIES

  2. FILL IN THE FOLLOWING BLANKS:

a. FROM – YOUR NAME AND ADDRESS

b. YOUR NAME AS AFFIANT

3. TAKE YOUR 7 AFFIDAVITS TO A NOTARY PUBLIC who will verify your autograph in the VERIFICATION SECTION where you will sign in front of the notary and they will then notarize each copy of your affidavits. Notary’s may be found at your bank, county government offices, some libraries, select printing companies such as UPS, FedEx, Staples, etc. You should not have to pay for notary at a government or your banking institution.

4. Scan one of the signed AFFIDAVITS with a scanner or a phone app. If you do not have access to a scanner, we recommend downloading Adobe Scan on your phone. 5. You will have 6 AFFIDAVITS TO MAIL.  Mail via US Postal Service CERTIFIED SIGN RETURN RECEIPT the ORIGINAL SIGNED NOTICES and send one of each to Governor Brian Kemp, Representative Randy Nix, Senator Max Burns, Lt. Governor Geoff Duncan, Secretary of State Brad Raffensperger and Attorney General Chris Carr (addresses are on your document) Keep one document for your records.

6. EMAIL a copy of your scanned Affidavit by attaching it to your email and address it accordingly:

In the BCC field copy and paste the email addresses below.  This will send the Affidavit as an individual e-mail to each House/Senate Member, Attorney General Christopher Carr, Secretary of State Brad Raffensperger and Lt. Governor Geoff Duncan. (Governor Kemp does not have email)
